Hi All, does anyone have recommendations for a cardiologist and/or Clinic (for INR testing) in southern Spain (Malaga)? As some of you may remember, our 19yo son had his OHS (AVR) at Cleveland Clinic last December. He is doing fine. And planning to study abroad in Spain early next year. While we don’t anticipate any issues we would like to have someone in country who could see him if needed. Thanks in advance!

He did not get the repair he wanted but did get the replacement he needed. He would have been discharged several days ago if not for delays in getting to therapeutic level of Coumadin. Wondering if others have experienced this?

As young as he is at 19 years, he will heal incredibly fast. I also live in VA, and had my surgery at Mt. Sinai in Manhattan, and then went to Inova Fairfax Hospital in Northern VA for my cardiac rehab.-- Probably the best in the DC-Northern VA area. Sharing this latter info, just in case you son is interested in the rehab. Even if he goes to only a handful of visits, instead of the full monte, might be helpful with his confidence to exert, etc.

Rita Savelis Yes. It can take days/weeks to get your INR in place. It varies from patient to patient and it is normal as your body has to get used to this new med. And INR will continue to go up and down, that's what it does - it is not stable. It can make one very nervous at the beginning. It's been almost 9 years for me now. I worried a lot for the first year. Now I rarely think about it, get a blood test when I feel like it (about once a month), decide on my own med doses and eat whatever I like (in moderation). As I said, INR will go up and down but usually eventually settles as time goes by. It's hospital protocol to not let you leave until you get to the range they want for you. You can't speed it up. It will happen as it happens. It's hard I know. I feel for you. It's hard for the patient. And harder in a different way for family. Wishing you the best.

Although I didn't have an artificial valve, my surgeon prescribed blood thinners for my first three months of recovery and I was surprised at the initial fluctuations. Your son will acclimate quickly just as Rita described and now he has a healthy heart and long- lasting repair. All the best to you both!

I'm posting here as a parent (not a patient) of a 19 y/o with a bicuspid aortic valve. My wife and I have encouraged him to join this amazing community. For now, I remain content that he appreciates it as we share the tips, tricks, and amazing success stories we have read from so many of you. Can't tell you how helpful this space and been for us as parents sharing this journey with him.
Unfortunately, he has reached the point at which he can no longer put off surgery. He is now scheduled on 12/21/23 (heck or a way to spend Christmas) at Cleveland Clinic.
We will be traveling from Arlington, Va. And I have a question and request for recommendations to those who have been to Cleveland Clinic regarding lodging. Where should we stay? We have stayed at the Intercontinental and appreciate its convenience, but prefer benefits of kitchen and washer/dryer. Reviews of Residence Inn are not that great but there is little else within walking distance. Any thoughts on Residence Inn? Also, there is a small apartment building next to the Clinic's parking lot#1. Pictures and reviews seem great (http://www.airbnb.com/rooms/44630288?adults=2). Google reviews, however, are terrible. But it is super convenient. Please let us know if you have any info on this or apartment building or any other good recommendations.

They took amazing care of me. I can't really help with anything other than a hotel as far as recommendations go because I was quite fortunate to have my Sister in law who lives 30 minutes away and opened her house up to all of us. Light headed and close to fainting. Unfortunately, I blew through my own blood in my first operation. I had a total of 3 procedures done in 48 hours due to a bone bleed in my sternum then a blood clot in my right lung? I ended up using a lot of blood that wasn’t mine.

My son has bicuspid aortic valve. Surgery tentatively set for Dec 2023. Initially told the surgery would be repair, and if not repairable then Ross before going to mechanical if Ross failed. But now we have new scan results suggesting pulmonary has slight leak so Ross appears to be an unlikely option. Repair is clearly our preferred outcome. Hoping this community can provide recommendations for surgeons with high level skills in aortic valve repair based on recent experience. PLEASE, please share your recommendations.

Grace Mason Hi Lyle, I have a small leak on my pulmonary valve, which I don't believe is uncommon. My surgeon believes I am still a candidate for Ross if my valve is not repairable. He does approximately one Ross per week in a year. Lots of experience. I am very impressed with what I have read about him and after meeting with him via "Zoom". I would recommend a consult with him. Dr. El-Hamamsy at Mt Sinai, New York.

There are excellent options for surgery and you can take your time to evaluate them. There's a very experienced aortic valve surgeon in Florida, Dr. Joseph Lamelas, who has performed thousands of minimally-invasive aortic valve procedures. You may want to check him out if you're willing to travel that far. I would be remiss if I didn't mention my own surgical facility (since you're considering the Philadelphia area), i.e. , Lankenau Heart Pavilion- Main Line Health in Wynnewood. Lankenau's surgeons and teams provide very personal attention and progressive approaches to cardiac care. Best of luck with your research.

You may want to get full testing done then consult with a few surgeons you’ve chosen to see how they would handle it. As you probably know, Cleveland Clinic has been tops for many years but there are many excellent surgeons.

Deb Michalowski Lyle, heart valve issues can be very scary, but finding the right cardiologist and cardio surgeon can put your fears to ease. I live in Scottsdale, AZ and my surgery team is not listed on this portal, but I researched the team I have to see if they went to good medical schools, have outstanding patient ratings, and asked friends who know of my doctors for their knowledge. The northeast has excellent doctors too with outstanding B and C Teams. Since you are in Arlington VA, I would research top docs in your area. Heart disease is the number one disease in America so there are many great cardiologists and cardio surgeons out there. My last requirement of my doctor team is they need to have excellent communication skills. I feel very comfortable with my doctors and I know I can tell them any of my concerns and they will address them to my satisfaction. This site is a great resource, but don't limit yourself to this only. Best of luck and happy to help more. I am on medical leave (I cannot work with my stenosis symptoms) waiting for surgical aortic valve replacement for my BAV that is failing with stenosis which was identified in 10/22 echocardiogram.

Allen Carkner My surgeon, Dr Vaughn Starnes at Keck Medicine of USC in LA was excellent. I had a Ross procedure in March as a 24 year old. Dr. Starnes works at Children's LA on children and infants and at Keck. I believe he is a good option for repairs and other valve options.

Hi all. I'm new to this community. As parents of a teen born with BAV my wife and I find ourselves facing what seems like a daunting challenge to make the determination of when to move forward with surgery, what type (repair/replace) of surgery, how to select the best surgeon for the type of procedure, and many more items I'm sure I don't yet know to list. Our son is asymptomatic though tests reveal regurgitation that we have been monitoring for several years. And with all of this we find ourselves a bit overwhelmed. Our son's cardiologist seems to think we should move forward with surgery soon (this year or next) to avoid stretching and/or other complications. Does anyone here know how we would go about finding a good surgeon for BAV repair in young adults? How do we find out how much experience a surgeon has with BAV repair and what the success rate is?

Rose Madura Hi Lyle, now I know how my parents felt when they were told I would someday need surgery when I was 7. This was in 1963 before they did these types of surgeries. I'm 65 now. I didn't need surgery until 2017 at age 59. At any rate, I would first suggest a second (or even third) opinion. I got 4 of them. Then, I'd do your due diligence in trying to find a surgeon who does repairs. Also, he may be a candidate for the Ross Procedure which our founder, Adam Pick had. You may want to read Adam's book. It is most helpful. You may want to consider a consultation at the Cleveland Clinic or a reputable Children's Hospital. Please keep us posted. This is the place for you. I was diagnosed with severe stenosis and moderate insufficiency (leakage) when I was 12. Played every sport, weightlifting, no issues. Never informed about the diagnosis though and was rediagnosed at 30 with severe stenosis & severe insufficiency.

Now I'm 32 and my Cardiologist believes I still have years before a replacement is needed. Really depends on how the left ventricle is handling it, I would definitely get more opinions.
The first opinion I got suggested replacement immediately, which in hindsight wouldn't have been the best option for me.

I can't really know what emotions any of you are feeling as you face your own journeys. And my son, who is the keep it to himself stoic type, tends not to share how he is processing this news. But maybe, as I walk this journey with my son and continue to interact with this community, I will gain a better understanding of what he may be facing. He is at college now in Carlisle, PA and we hear that U Penn has some good surgeon so we will be setting up some appointments for second opinions. I'll keep posting. Love to all of you!

For example, starting with an AVA of 1.22cm^2 almost three years ago and most recently having an AVA of 1.67cm^2... 31% difference in a good way.
The peak/mean gradients have followed suit as well and my left ventricle has gone from dilated to 'normal'...
