Dental work and preventive antibiotics?
Next week I'm scheduled for my first dental checkup and cleaning since my Ross Procedure 9 months. ago.
My dentist has asked about inquired regarding prophylactic antibiotics and my post-op situation. My surgeon's office has given me guidance about this, nonetheless, I'm interested in hearing from other patients about what their doctors have recommended or prescribed to them. My operation was deemed successful. A Ross involves switching out my own pulmonary valve for my calcified bicuspid aortic valve, and replacing my pulmonary valve with a human donor valve. No synthetic materials or animal tissues were involved.
I'd appreciate learning about what protocols, if any, were/are still required and what are the timelines, i.e. months/years /forever regarding antibiotics and routine dental work. Input from people here would be much appreciated.

Richard Munson Inwas told after my surgery to expect to take antibiotics forever before dental work. Due to bacteria in the mouth that can affect your heart. My dentist concurs.

Susan Lynn The American Heart Association puts out guidance on pre-medication that has changed over the years. I've been pre-medicating since my mitral valve prolapse identification 40 years ago. At one time, I had to take antibiotics 3 days before and after my dental appointment. Today, I just take one large dose an hour before my dental procedure. I think it's a small inconvenience for the protection it provides. ❤

Carlos Banuelos Have always pre-medicated both since discovering my heart condition and post-surgery. My doctors all agree it’s better safe than sorry, given that I don’t have any allergies to antibiotics.

Rita Savelis I take antibiotics one hour before teethcleaning. The dentist always asks if I've done that and my cardiologist or dentist writes the prescription. I was told to start doing this when the stenosis of my aortic and mitral valves was diagnosed (6 yrs before replacement of both valves) and continue to take antibiotics an hour before treatment 6 years after OHS and indefinitely.

Martin Smith Thank you Richard, Susan, Rose, Carlos, and Rita. Your responses are helpful. The "forever" word, when I first heard it in response to my situation, came as a surprise, and that's what prompted me to ask for views from here.

Lindsay Beth I had surgery to repair (not replace) my pulmonary valve when I was a baby. The only medication I took was amoxicillin for the dentist. Later, there was some research that came out that said if you have your original valve you do not have to take the antibiotic. I talked to my cardiologist about that and he said that- out of an abundance of caution - I should continue to take it. He told me that it was really the most sensitive area in terms of infection. I was told that after my heart surgery the risk of infection diminishes significantly after a year or so - that I should plan on zero dental work unless absolutely necessary for about a year. After that, the pre-med antibiotic would be compulsory. I know it's a giant dose of antibiotic, but I do think it's recommended. My dosage is similar to Rose.

Martin Smith Lindsay Beth, I just read your post having believed that all responses were in. Thank you! Interesting about the 1-year avoidance recommendation. I've now been twice for cleaning and did take amoxicillin 1 hour prior to the appointments. Good news: I'm still standing. Touch wood!

Kimberly Eisenhut Im so glad I read this post. I have tricuspid regurgitation and have not had surgery yet, but every time I ask my cardiologist about this, she says it is not necessary. I really do not undrstand why I am not being given antibiotics before any dental cleaning or cavity filling. I will now demand antibiotics before seeing the dentist. I have brought the subject up so many times with both my PCP and Cardiologist, it doesnt make sense !?

Katheryn Applegate This has been an on again/off again recommendation for prophylaxis. Currently not recommended except for those of us with replacement heart valves. Dentists won’t proceed without it and cardiologists do recommend it for us. 1 megadose 1000 mg I believe, 1 hour prior to dental care for life. Annoying but really no big deal.
