A year ago this morning I was doing my final skin cleanse before walking in a glassy-eyed stupor a few NYC blocks with my wife to my surgical appointment at Mount Sinai. I don't miss that moment, but I'm grateful beyond words for all the things that have gone well, gone better than well; and for the challenges and disappointments through which I continue to learn and grow. It feels like a new year for me... like somehow this anniversary is a rebirth... that like much of this journey through the years, is another lonely and sober moment even still, despite all the love and strength and support of my friends and family. I know you all can relate. Glad to have you all - even just knowing there are so many out there who understand, who walk that same road: all those I have never, and will never even meet; but whom I love and cherish nonetheless.

No fevers this time, but debilitating and relentless, and difficult to sleep and move. I've been taking it easy and can't point to anything that caused it this time (vs exercising the last times). Have any of you, or anyone you know had struggles with this? It's hard to diagnose and my first recurrence went unrecognized. I'm just trying to figure out if this is a relatively normal part of the recovery process or if my cardiologist and I need to start pursuing unappealing treatments like long-term steroids. :-(

Six months ago today I was just waking up. Everything about that surgery and how my outcomes have been so far have been better than I could have ever hoped. Life feels back to normal in most ways and I haven't had any lasting complications. The road has been very bumpy at times, bumpy enough that I'm not near as far physically as I was hoping and expecting to be, but that never stopped any of us. So thankful to God for everything he's brought us through, and so grateful for these little squirrels who have been so thorough in their efforts to challenge the strength of the sternum wires...

Has anyone here ever had about a pericarditis after surgery, or some other pericardial syndrome? This is the second time now in 6 weeks with eerily similar symptoms of fever, elevated inflammatory markers, pleuritic chest pain, left shoulder pain... All of it getting worse when I lay down at night. Last time my fever was much higher and there was some sort of viral component ultimately, but still no infectious symptoms. My understanding is these things usually happen closer to surgery and I'm I'm 5 months out, but I still have a pericardial effusion so who knows.

Patricia Miller Matthew- I experienced some of those symptoms. I had vague chest discomfort/pain about a month after my robotic MVR. I was told it was costochondritis and to take tylenol and ibuproph. My chest pain was more w/ leaning forward which is different than typical pain with laying flat. CRP was elevated and was told to recheck in a month. Very high at next check and then was told I had PPS the whole time (post pericardiotomy syndrome) and to start colchicine. Had a couple pulses of steroids which helped w/ symptoms but steroid use can create rebound concerns.. My initial cardiologist only prescribed short term colchicine for the pericarditis but when I switched doctors my new cardiologist prescribed three months. Three months was standard per my surgeon's PA and a cardiology NP I know. My symptoms resolved 6mos after surgery which was about 3 1/2 mos total of colchicine. No pericardial effusion though. Have you started a course of Colchicine? For how long? From what I read, pericarditis can rebound if not treated long enough. I did learn that using the word "discomfort" doesn't always get the attention that "pain" description does. So don't hesitate to speak up. It is all frustrating when you are more than ready to feel better at this point. Especially when you've worked hard at recovering. I did not have fevers like you have had. Colchicine was easily tolerated for me. No side effects. I hope you are feeling better soon.

Matthew Malooly Thanks for the well-wishes. Still waiting on cardiology's review of echo, but I am probably in the clear. After never having a fever in 15 years and no sick contacts, I had a high fever >103 for 3 days with no other symptoms. Labs showed high systemic inflammation and stress on nearly every organ system: liver, kidneys, lungs, heart... even with an elevated d-dimer needing evaluation for pulmonary embolism. I ended up getting admitted for empiric antibiotics until endocarditis could be ruled out. In the meantime, my youngest (18 months) spiked a fever of 104 with no other symptoms... which honestly put us all at ease. Every other test, including my gross evaluation of the echo report, says I'm in the clear for now, and I've only had high-normal temperatures since. I think this was all just a pretty wild viral syndrome from who knows what.... thankfully I'm young and otherwise healthy. This one hit hard. Still pretty sore and still get tired really easily (I feel like I did two months of recovery ago)... but I still have a little fluid on my lungs and heart. Otherwise doing well. Overall, I was so grateful I had just gone through heart surgery in January... first because I was prepared for dealing with stressful and overwhelming situations like this and felt like I remained calm and optimistic the whole time... and second because we already hit our out-of-pocket max, so this was all free! :-)

Hey Y'all looking for some advice on recovery:
I'm coming up on 4 months post-op. Other than the bit of weight I gained from the otherwise wonderful sedentary time, I'm feeling good with basic life stuff and have had no complications. I've been at work now for a month and am happy that I no longer have much pain or fatigue just from the daily grind of housekeeping and working full time.
Since getting back into the routine, I started having palpitations at a pretty normal rate for where I was at pre-op. Cardiology has been aware of these changes and hasn't been concerned. A post-op echo shows everything is great and my heart is starting to remodel back to normal size already! I got the go-ahead for returning to a more normal exercise routine
For several years preop I successfully targeted a total of 150 minutes/week of moderate to high intensity cardio with HR > 160 bpm about 70% of the time. During recovery I stayed walking, but my daily routine has slipped since I've become more busy and more active as well. Last week I started trying to ramp up my exercise and was able to get a few leisurely sessions, and one where I actually broke a sweat and got my heartrate up over 160 for a pretty good amount of time. It felt great to push and I'm ready for more. But in the days that followed I had hundreds of palpitations that took a whole week to finally get back to baseline. Cardiology advised me to slow down and be more progressive, but this is where I need help.
I'm very discouraged how long the tail end of this is lasting. But of course I gather that's normal. I don't know how to be more progressive. It's hard to stay motivated to continue to work if I don't have the sense I'm gaining ground. Any tips for how I can work my way back progressively in a way where I can see consistent results and feel I'm making good effort consistently? I've found it difficult to do really ANYTHING if I can't engage with the strenuous and meaningful work that's been so important for my psychological and physical health over the last 5 years.

I realize you are much younger than I and can't wait to get back to normal but it will take time. Granted, I had tons of complications but I really didn't feel "better" for a year, I'd say. Try not to compare yourself to others as we are all different. It will come, just takes time. You had a major jolt to your entire body. Godspeed.

Luke Rodriguez This is probably not what you want to hear....I am just shy of my first anniversary of my surgery....aortic valve replacement and partial aorta replacement....all mechanical. It is OK to slow down. This is a relatively brief moment over the course of your life. I have a sister who is a nurse and a niece who is a cardiac nurse, both told me to take things slow. My cardiologist said go slow. Finally, my body said slow down. The problem with me is that I felt awesome going slow....the more intensity I added, the worse I felt. When you hit the six month mark you will see remarkable improvement.....I still have twenty pounds to get rid of, seems like I could drop weight easily before surgery, now not so much. Today I am feeling better than I did prior to surgery, been doing just about anything I want to do...weight lifting, cardio, lots of hiking...did a 10 mile hike a couple of weeks ago. Bottom line is, be patient, you will come back from this. Keep it up.

I am at 6 months and have very gradually added more and more but sometimes I still over do it and my body really lets me know for about a week (usually light weights related). My surgeon told me to expect a full year to get back to normal and to pace myself accordingly. The best advice I have received is to listen to my body. If you had such a reaction in your heart to pushing that hard, then you have to slow down a bit. I had similar heart feelings if I did too much cardio as well a couple of months ago. I called my hospital hotline and they said I was doing too much and to back it up

Klara Čičić Hi Matthew, I am 7 months post-op and a month ago I started doing yoga and cycling. At the begining of each excersize I have palpitations. As I go on with excersizing they become less. If I strain myself too much (cycling uphill), there is quite lot of palpitations. Then I try to calm myself down with breathing and slow my pace and sometimes it helps. Overall it is unpleasant. I also wonder if I am doing too much. But how will I ever improve and progress if I don't push myself? It's frustrating. But I know I'll keep going, I want to. You check with your cardiologist, maybe do long term ECG monitoring during and after the excersize to see the nature of those palpitations and then ask him for advise how to proceed, what to do..... I have all of the side effects - weight gain, difficulty losing weight, lethargy, exercise intolerance, sleep disruption - after increasing my dose. But I'm happy to live with it for the price of getting my sutures epithelialized (I was an aortic valve repair)
Klara - did you have that many palpitations before surgery too? I never did cardiac rehab since I don't have anything very close to me and I didn't have any issues whatsoever until I got back to work. Maybe it would be a better idea now - even if just for the monitoring. ... Read more

Pamela - I've noticed the same thing - everything feels totally fine and great in the moment - then you pay for it the next few days. Everything from doing a lot of housework to eating a bit too much.

Klara Čičić I did. Always was prone to those unpleasant PVCs, and since I am very skinny I feel them each time. It's also PACs. After the surgery maybe I have less of them overall. I am on beta blocker bisoprolol 2,5mg daily. My only side effect is dry eyes. Can't wait to be able to reduce the dose by half. Hopefully with time. Now I am focused on building strength. My body was always weak as I didn't do any sport.
I can totally relate to your words about world seeing us as healthy and recovered... This is amazing community. Helped me tons and still does. Maybe don't blame your drugs on everything, hope you have a quality GP (PCP) to check what is really going on or is it just the meds+recovery... Keep us updated...

Picture from my last day off yesterday. Back to work this morning. I have to say I struggle to find a part of me that's excited to be back. Maybe only for what it represents, that finally for the last 2 weeks my mind and most of my body feels somewhat normal again. I truly believe going back to work and getting back into the too busy lifestyle is the next most important part of my recovery. But these 12 weeks have made me really consider what I want to do and what I want to be, and we'll see. It's wild to me just how much of our life gets turned to glass just based on the amount of time we spend doing the work we have been chosen to do. This isn't one of the best days, but I cannot forget how many worse ones have come before. If we can do that, we can do ANYTHING.

I went back at 12 weeks, and it was rocky for 3.5 months (very physical job) and my wife and I decided it was not worth it anymore. I have since started my own business and work from home, I drop off/pickup my daughter from school, have dinner ready for my wife when she gets home. It has its ups and downs, but overall, Best decision I’ve ever made. Priorities change. Life changes. What’s important once may not be important now, and what may not have been so important, is very important now. Hope this helps!

Richard Munson I once thought retirement from sales was the greatest thing to look forward to since sliced cheese. Lots of stress. How wrong i was after a month. That was 15 years ago and lucky me being self employed i was able to jump right back into the fray. Now at 81 and mitral valve repaired, i am happy for the decision i made. Keeping in mind, we are all different, i still look forward to the battle every day. And today i had a doozy but i brought the various parties together, worked out a compromise and now i am sitting back fat dumb and happy ( and kept a customer). Many of my retired friends ask me why i still work. My answer is i enjoy it. This is not meant to change anyones minds, just my perspective.

I was fortunate to be able to retire (willingly and from a sales manager job) when I needed my surgery. I'm not sure I could have gone back. In a way, it's good to be back, but at (almost) 63, I would like to expend my energy doing something less strenuous. I hope to retire at 67, but not sure what I'll be doing until then!

Klara Čičić Good hearing your thoughts, guys. I can relate to "changed perspective" after surgery. I started working very excited and finally feeling like myself again. But I struggle with keeping the stress level low, since I am quite energetic person and passionate about my job as a PCP. I mean, the only thing I could do is change myself or change job. For now, I am focused on changing myself haha, minimizing all unnecessary stress, getting helpat home, I started doing yoga so that may help... Matt, 3 months is still early, I am sure with each day you will feel better phisically as well as mentally:)

Just sitting here ready for discharge tomorrow morning. All tubes removed, finally pain free without narcotics, feeling rested and comfortable. I know there will be more challenges and bad days in the months ahead, but I'm allowing myself to enjoy this moment of bliss just sitting watching football while my wife is out enjoying NYC with my brother and sister-in-law. These are the moments to hold on too, and to look forward to. We are stronger than we know!

Robert Miller You look great! Getting all those tubes out makes a huge difference. Take it easy for the next month or so even if you feel like you can do anything. I made the mistake of taking all the Christmas lights down and putting all the Christmas stuff inside the house away. This was a little bit less than 3 weeks after surgery. I was working the whole day (it actually takes two days total) and lifted more than 5lb multiple times. The next morning I had funny noises coming from my chest (sounded like fluid on my lungs). Luckily 3 days afterward things were good again, the x-ray didn't show anything on my lungs and no broken wires (the ones that hold your ribs together). My surgeon thinks it was fluid that did build up (outside the lungs) and was eventually removed by the body. But as I said, my point is, take it easy and follow what your doctor tells you.

Very unfortunate, sorry to hear about this. I've done some reading on the nursing reddit and assuming what they're saying is true, the patient to nurse ratios sound insane at Mt. Sinai. Seems to be a primary motivator for the strike.

I can completely understand and relate to everything they're complaining about: patient ratios, ED boarders, wage stagnation, open positions, chronic understaffing predating 2020... but I have no union. Hey y'all! 27 days until surgery and I'm trying to get planning done before Christmas. We'll be flying to NYC. I already have a VRBO and flights booked. But I'm trying to plan what to bring on the trip and what to get ready for home. I'm ready at home with a recliner and a treadmill, but I'm worried about forgetting something that will make life easier when we get there, since we'll have no way to get it. My main worry is there isn't a good chair in our VRBO, so I think I'll be propping with pillows for the 5 days they want me to stay in town after discharge. Any suggestions or lessons learned that you can offer? I need your wisdom!

Cheryl Mann I purchased a wedge pillow - it was actually 2 wedges - one for behind me knees and one behind my back - they allow you to rest/relax/sleep in a position that is somewhere in between sitting and laying flat. Add in fluffy pillows to prop up your arms and that is pretty much the best you can do in the beginning. I also listened to ocean sounds to help me relax.

Which hospital will you be at? A few things that are helpful in NYC: wash and fold laundry picks up and delivers. Try to find places to shop walking distance to the rental and most likely they also deliver. Also use the incentive spirometer they give you at the hospital after the surgery. It’ll help open up your lungs. I did not have a recliner and I was fine log rolling.

Was told it is important tool in building up your lungs and coughing out mucus and what ever else builds up during surgery. I used it religiously right after the operation as well as home. Yes, it makes you cough but it does aid in clearing your lungs. My mgh roomie and i used to have a contest on how high we could make the little ball go up. Then after we groaned from coughing for the next 10 minutes.

Thomas Brusstar My mitral repair with a regular sternotomy at age 54 was fairly easy, I was using plain tylenol by 48 hours afterwards. I went home about 72 hours after. I didn't need a recliner, I was able to lie down on my back and sleep ok. Showers felt good, and I was able to stand fine. Just reaching around too much to dry off and stuff was tender, but shower itself felt good when I got home. Bring a super long phone charging cable, so you can plug in even when you're in the hospital bed. 3 feet is too short, get a 6.

Klara Čičić Hi, I travelled 9 days after my mitral valve repair done via right mini thoracotomy. It was a long trip, by car 5-6 hours divided by one hotel sleep over. I was taking a nap in a car so I needed a few large pillows and a blanket. Besides those classic hospital stuff lists,my essentials were: warm ski socks (I even wore two pairs of wool socks at once, your circulation those first days is weak and your periphery is colder), ear plugs, artificial tear eye drops, sleeping pills, long phone charger. I needed no recliner.

Also, keep the showers short and warm, not hot. Prolonged, hot showers can drop blood pressure. I didn't find out about that until cardiac rehab. Take care - you'll see progress very quickly.

January 18th I'll be making the journey with my wife to Manhattan to be under the care of Dr. El-Hamamsy at Mt. Sinai for repair of my bicuspid valve; with intent to save the Ross procedure for down the road. This has been a long journey over most of my life and I feel one of the hardest parts if finally over. Paige Hahn Hi Matthew, I read your posts and I really hope the nurse situation doesn’t derail your surgery! I am scheduling an appointment to speak with Dr. Stelzer about a Ross procedure. I saw that you will do a valve repair and not go ahead with the Ross. That’s great news! I think my valve is too compromised for a repair and maybe the best solution for me is a Ross. Can I ask you if you feel confident that Dr. Stelzer is an excellent doctor for a Ross procedure? And did he give you more information on how long the valves would last? Or if you were given other useful information? Thank you so much! Be strong and you are in my prayers and thoughts!!!!

I'm two days out from my initial surgical consult. Any tips on assessing a surgeon? I work in hospital medicine so I know the kind of scary providers that are out there. I'm traveling two hours from home to go to University of Wisconsin (based on a second opinion 14 years ago when I was initially told I'd need surgery). I was referred to a surgeon who has only been practicing 2 years, but I know that means he's been doing it for a decade. His educational credibility is top notch (Stanford, New York Presb), but this is something that has to last me a long time.

Otherwise, there should be stats on how many of each procedure the docs do every year if you google the surgeons or contact the hospital for that info. High volume is a good thing.

Thomas Brusstar I've heard from many that pure number of procedures is important. Maybe if he is relatively young it won't be as big a total, but you want a surgeon who does your surgery several times a week. My surgeon had done more than 10,000 surgeries, including 4,000+ mitral valve surgeries.

Matthew Malooly This would actually be my first surgery. I was originally seen by pediatric cardiology. When I turned 20 there was a significant enough change that they referred me to adult surgery, who decided I was not yet eligible. I've since been managed by an adult cardiologist. I'm in healthcare so I know how things go with sometimes I think dangerous providers hiding in plain sight, and no one knows except the staff who work most closely with them... and even so would likely never suggest that at all to a patient. It's hard for me to go to a place I'm not familiar with when I rely on that inside knowledge so much for myself and my family with my normal healthcare network. This feels wrong. There should be a better way for patients.
