Its been a while since I checked back with the community here. Hard to believe it has been 8 months since I had my OHS and by the grace of God things have progressed very well. I am back to my normal routine and back to 10K runs and other cardio-physical activities. My work travel has taken me all over the world in the past few months and it has not been impeded in any shape due to the surgery. I am thankful again to God for every single day that he has bestowed upon me and have a whole new appreciation for life and more importantly a different and more meaningful perspective on life. I realize the apprehension that one goes through as he/she is about to go through this major surgery. My well wishes to everyone who is about to embark on this journey. Stay positive, stay strong, and don't hesitate to ask for help.

Question regarding effects of exercising and warfarin - My INR level was pretty stable for about couple of months but noticed that it has been dropping in the three weeks or so (my dosage is being increased to counter this drop). I am starting to regain my strength and exercise regimen dating back to pre-surgery days but noticing a lower INR reading. Is their any co-relation between exercising and its impact on INR levels. I have read contradicting info on this topic and wonder if folks have any insight into this. I just got my home monitor couple of days ago so that should help.

Wanda Mroz I myself was on warfarin for three months. I don't recall my starting rehab and increasing my activity making much of a difference in my INR. It did change now and then but activity did not seem to be the cause. if that helps. I'm no longer using it though.

Hello to all my fellow heart community folks. I am coming up on my four months after OHS and reading some of these posts brought back lot of memories as I was about to enter the most difficult part of my life. Just some friendly advice for those who might be staring at a surgery in the near term - 1) Do your homework; ask as many questions as you can and don't even hesitate to ask some of those questions again; 2) Get a second opinion and last but not the least 3) Stay positive - as hard as it seems a positive mind will go a long way in helping you get through the rough patch.
By the Grace of God, my recovery has come along well and I am thankful to everyone's well wishes and prayers. I completed my 5K run last month just before Christmas and planning to do 10K before the end of this month. Starting to regain some of the weight that I had lost after the surgery.

pino correale Hi Tapan, i am happy you're doing so well. It sounds like you are a runner like me. I am not quite 8 weeks post/op AVR and i have signed up for a 5k run on June 5th/16. I hope i shall be ready by that date. My recover is coming along well, no major complaints. Just in the beginning i had a tough time getting a decent sleep, but with a little help from my doctor , i seem to be managing fine for now. Physically i must say i did amazingly well, less so on the emotional side.....but i am on the mend. After all as you know OHS is not really for the faint at heart. Good luck on your 10 k run.

Hello Everyone. Today marks 8 weeks since I had my bicuspid aortic valve replaced and aortic aneurysm repaired. Hard to believe how quickly time is zipping by. By the grace of God, my recovery has come along very well and for that I am very grateful. I started work couple of weeks ago and my exercise regimen continues to get better by the day. In fact just yesterday I was able to run (albeit slow) a mile and hope to keep bumping up that number. Before surgery I had set a goal of running 5K before Christmas and hopeful that I can do that. I hit the gym for the first time today and was able to start light weight workout. I also want to take this opportunity to Thank various folks in this community for their guidance leading up to my surgery. I also want to convey my best wishes and prayers to all the folks who have their surgeries slated over the next few days. I know its not easy as one gets closer to the date as I certainly went through my highs and lows.

First and foremost Thank You everyone for your prayers and well-wishes in regards to my heart surgery. My surgery was successful whereby they replaced both the leaky aortic valve and the aneurysm (got a composite valve graft). Dr.Craig Miller and his team at Stanford were outstanding and made my stay very comfortable. Nurses, physicians, etc all of them were superb! I got discharged after five days and have been recovering at home. My recovery is coming along quite well and I have been walking about 3-4 miles/day for the past five days. Saw my cardiologist and surgeon today for follow up and all looks good. In fact my surgeon asked to bump up my walking to about 7-8 miles/day, which I plan to start tomorrow. I am glad the surgery is behind me and now squarely focused on recovery. I am greatfull to the God above for guiding me through the most difficult period of my life and for giving me another new lease on life, which I plan to make more meaningful from here on.

I will be going with a mechanical valve at my upcoming Sept. 23rd surgery. It is my understanding that some insurance companies have a 3 month waiting period before they will pay for a home monitoring INR machine/kit (don't quite understand why). My question for the community is two fold - 1) How often did folks test their blood in the first couple of months after surgery and 2) Did you go to a Coumadin lab or had someone assist you at home (i.e. nursing service) with regular blood work the first couple of months?

Clare Auten My hospital has an anticoagulation clinic but I don't actually go there. I go to a lab right by my house. They send the results electronically to the anticoagulation clinic and I get a call by noon or so the same day. The amount of time between blood draws usually depends on my INR. Once I got into range, they started spacing things out. Also the girls at the anticoagulation clinic are pharmacists who are always available to answer questions about interactions and stuff

Steve Salvador your cardiologist should give you the info about a anticoagulation clinic that they use. They work side by side. Anti clinic is better because they prick your finger tips, instead of a blood draw from your arms. Alot draws from your arm is not good. If you plan on being on warfrain for the rest of your life, they told me it will be better to have your own machine to check your levels, they have some that will electronically send it to the lab and give you info in seconds. For a few couple of weeks they will draw weekly, then every 2 weeks, after that every 30 days. It depends what you eat, that is very important, and in moderation. If you smoke and drink alcohol, I advise to quit. It will affect you PT/INR levels. BTW, I went with tissue, I didn't want to fool around with drawing blood and watching what I have to eat..... Rita Savelis At the hospital (I stayed 4 weeks) I had blood tests frequently and also anticoagulants through IV, but now I go to a lab once every 2 weeks. I can check results on the internet. It's easy and the technicians are nice. I eat everything I've always eaten, but I don't drink a lot of alcohol. I phone my doctor if I can't figure out what dose to take but after awhile it becomes easy to figure it out on your own. There is definitely a few months of stabilisation at first where you think you'll never find a correct dose. I understand that the liver has to get used to the meds.

Jean Raber Three things that might be helpful to add here. 1) Contact your insurance company now to see if they will pay for an INR machine. If not 2) ask your doctor if there is a visiting nurse service who can come to your home and get your INR numbers. If the answer to both questions is no, explain the situation to your doc. If he has no answers 3) see if there is a volunteer organization (Council on Aging or the Red Cross) who would provide a volunteer driver to take you to the coumadin clinic until you can do so yourself. You may need frequent monitoring (every two days) after surgery until you can drive again. Best of luck.

In my experience the at home inr is not as reliable as going to the labs. Your going to have to watch your diet fromthe very start and depending on your inr and how fast you get it regulated, will determine how often you will need to visit your lab. Elaine Cutri I got INR tests about once a week after discharge, then when the results were consistent for 2 tests, they stretched it out to every 2 weeks, then once a month. If the results are out of range, then it's every 2 weeks. I go to a lab near my house for finger pricks. Do you know if there is a lab near by? Terry Shermeister I'm a teacher and my friend had the mechanical valve put in. He's had it for nearly 15 years, no issues. His only complaint is the blood work and thinners. I know how he stressed over it, but had he chose any of the alternative valves, other than the repair, he would have had to have another one done by this time. Only you can make this choice and it sounds as if you have determined what you will do. Great!

Well Sept. 23rd is almost here and my anxiety level continues to build, which I hope is normal part of this process. Still hard to believe that I will be going through this surgery especially given that I am asymptomatic. In fact just ran 5 miles yesterday and still feel like I have not lost a beat. Have mixed emotions but squarely focusing my resolve on post surgery rehab. Have bought a new pair of shoes that I plan to break in after the surgery with the goal of running a 5K in December. Plenty of prayers that all goes well for myself and for those who have their surgeries coming up.

I have a friend that opted for the tissue valve and guess what, he had some issues and he's not taking blood thinners. On top of that, he only 40 years old and will with some luck from god, he will need another surgery. I would never want to go through the open heart procedure again.

Ever since I found out that I need an elective surgery within the next couple of months, I have been struggling with a decision to go with mechanical vs. tissue valve. One surgeon suggested a mechanical valve and another surgeon (2nd opinion) suggested a tissue valve. On one hand I don't want to stay on Coumadin for rest of my life but at the same time the thought of going through a guaranteed re-op is not desirable either. I realize there is no ideal option here and at the end of the day, I will have to make a call.

Steve Offenbecher I struggled with the same decisions that you are dealing with today. After much research and 2 surgeon opinions, I elected to go with the bovine synthetic valve. The advancements in this valve replacement technology is advancing at lightning speed. Just 4 years ago my dad was denied the minimally invasive replacement because it was not approved and now it is state of the art. I plan another 25 years and so I expect a minimally invasive replacement in the future. I am now 3 1/2 weeks post surgery and I could not be happier. My valve works perfect and I walk 2+ miles every day. I never had more energy, but I will never be on Coumadin, just a daily baby aspirin. In the end it is your call, but in my case, I would not change a thing.

Kenny Knox I picked mechanical too as I already had a tissue replacement 18 years ago. My goal is to not need another OHS. So far I haven't had any issues with Coumadin. I eat what I want (I eat pretty healthy as is) and I've been dosed accordingly. I take a mix of 2.5-5mg doses for a weekly dosage of 27.5mg. Again, haven't had any issues.
Regardless of what everybody says, go with your gut feeling. The tissue valve that lasted me 18 years was great!

Pat Bluemel I chose tissue. Important to note that reop is not completely off the table w/ mechanical. There are folks on this site that are on their second mechanical (and some on homograft valves when mechanical and tissue valves didn't work out). It's a very personal choice and there are chances and risks that you have to weigh out with both of the common replacement options. My choice for tissue at 42 - I'm accident prone, I didn't want to deal with Coumadin yet, and I have full faith that when it's time to redo again (likely with mechanical) they are going to have TAVR as a common option. 1 baby aspirin required for me daily for hopefully the next 15 years with my Trifecta valve. I am very much at peace with my decision. Good luck with your choice.

Tina Maroon Tapan my Heart Surgeon at the Cleveland Clinic preferred a Tissue Valve for me if I needed one during my surgery. I was not sure why though. But Dr. Soltesz was able to reconstruct my Aortic Valve. So I did not need a new valve. Wishing you the best.

Hello Everyone. My name is Tapan Mehta and I am a 43 year old male who has followed HeartValveSurgery.com for several years but never really participated in the online community until today. I have known about my congenital aortic bicuspid valve for several years now (found out through a routine physical exam, which led to seeing a cardiologist) but it was not until last week where I was told that its time for me to have the surgery to fix my aortic root and valve. To say the least, the past week has been an emotional roller coaster as I try to sort out the next steps.
I need to get both my aortic root and aortic valve fixed and met with Dr. Craig Miller at Stanford earlier this week to talk about the surgery. I have two questions for the community members - 1) Does anyone have insight about Dr.Miller as it relates to this type of surgery? 2) Besides Dr.Miller, who are some other top surgeons in California who could perform aortic root and valve surgery?

Daniel Spurgeon I will say you should make your own decision. A lot of patients underestimate the skill of surgeons other than their own, and make comments loaded with hyperbole. My surgeon isn't the only person qualified for a David procedure, but I chose him based on his experience, bedside manner, and think he'd do a good job on any patient that he thought required surgery.

Like his own surgeon, he did a good job on him so why wouldn't he do the same for you? I went by word of mouth and my Thoracic Surgeon, to me, is the very best. He saved some of my friend's lives, and now my own.

Tina Maroon Tapan I I send you prayers and nothing but the best for your surgery. I had my Aortic Valve repaired instead of replaced during my Aortic Aneurysm Repair this June, 2015. Hoping they can repair yours and not have to give you a new valve.
